OCI Checklist

1. OCI Application Form;
   
2. Two Photograph;
3. Current US Passport;
4. Proof of Marital Status (if applicable);
5. Proof of Indian Origin of Applicant : Copy of First three and last two pages of the last Indian Passport held;
6. Naturalization Certificate (if Naturalized Citizen);
7. Landing Papers (if currently Canadian Resident);
8. Proof of Renunciation of Indian Citizenship: If you ever held an Indian Passport, it is MANDATORY to provide proof of Renunciation of Indian Citizenship along with your OCI application.
9. Proof of Address.
