We do not authorize Visa And More to disclose any information to anyone that may come across to them during the documentation process.
This form also advise Visa And More of the type of service you are requesting, the departure date of your trip, and the return address for mailing the completed documents, in order to properly process your requested service.

Visa And More will not be held liable for any financial loss occurred by delay in the process. Regardless of the result, Visa And More can collect its fees, once the service has been conducted. I understand that government fee, consular fee, printing, photostat, notarization and other charges will be extra.
